05/07/2026 mklose called 508-892-6000; I spoke toMcDavid Rodriguez . We have been attempting to get pictured of his V60 ventilators to prove he had the FCO69 completed. He was very rude and was going to call his boss because his boss told him he didn't need to give us anything and refused to do so. I gave him Fady Messak cell phone number and asked he call our Field Operations Director. he adv he would call his boss and call me or Fady back tomorrow.

3/5/26 K Clements - There is no bench FCO WO to show evidence of fco69 completion. This Case needs to remain open until photos can be provided or validate onsite and verified by Intertek. See 1/6 note of contact that indicated labels replaced as we need photographs to close.

09/17/2025 YEspino Called 508-892-6002 to follow up and if they devices have been repaired already so that we can service the device in question. Was unable to speak to him so i left a message.
09/04/2025 YEspino Called 508-892-6002 to follow up with Mcdavid over the devices in question. Was unable to speak to him or leave a message because while waiting for the call to be picked up the call was dropped.
07/25/2025 MHamersley - called 508-892-6002, spoke to McDavid, he stated devices were still being repaired and that the recalls should be completed on them while there.
05/21/2025 MNewman - Fady called customer to discuss the repairs possibly needed for this case on 2 devices. Called McDavid at 508.892.6000. No Answer. Called 502.892.6005 and 6002. One call went to the pharmacy. Fady called a previous contact and left a vm. Fady will be replying to the email as well and looping the contact in for update on devices.
05/21/2025 MNewman - replied to email to loop Fady and Sean into the thread. Asked customer in the email what repairs are needed for the 2 devices on this case, as we are not sure if they have been called into Philips.

09/26/2024 Rarnold - Inbound call from McDavid, 15088926002. He was asking about scheduling. He did state that the 2 devices are actually down and need repaired. I let him know we can not touch the devices until they are fixed. He will either call the repairs into Philips or have them done in house. Placing on hold for now.
