07/14/2026 ARoosa - Recvd call from 309-691-4005 Jenny at OSF Medical group Surgery Center and she said they are a clinic outside of the hospital and that they do not perform surgeries there they only schedule them as well as follow up appointments with physicians. She gave me number to the anesthesia group thinking they might be able to help us 309-692-5393.

06/09/2026 ARoosa - Recvd. call from 309-683-4840 Julie from OSF Breast Center returning my call, She stated they do not have this equipment at her location and stated it could possibly be at the surgery center or hospital. Suggested I send email to jcation@cfhasc.com and stated she wasn't sure if this is who to even contact. Looking at prior notes we know device is not located at Center for health Ambulatory Surgery center So I am going to try to call OSF Medical group Surgery at 309-691-4005
06/09/2026: ARoosa - Called 309-683-5522, Line just rang over and over so I disconnected. Called 309-683-4840 Breast center spoke to receptionist who I explained to what I was calling about she said maybe Julie can help and xfer me to VM of Julie Custer manager of Breast center, LM.

05/19/2026 YEspino Called 309-683-4814 to speak to the Surgery Charge nurse to verify the devices in question. Was able to speak to the nurse and she advised that she would take a messge for Gus and he would give me a call back tomorrow.
05/19/2026 YEspino Called 309-683-4488 to speak to the Surgery center to verify the devices in question. Was sent to the Materials dept but ws unable to speak to anybody so i left a message.

Was able to speak to Jackie who advised i could call the surgery center @ 309-683-4488 or the surgery charge nurse @309-683-4814 to be better assisted.
