06/03/2026 HWilliams - Called 331-210-2062 and spoke with Sergio to sched FSE Bennet for remediation of devices on 6/5/26. DE43811976 was replaced with DE43812388 under warranty so DE43811976 will need an ANR while FSE is onsite. FU with confirmation email.

05/19/2026 ARoosa - Recvd email from Sergio confirming all devices except for DE43811976, it has been replaced by DE43812388 we can call Katie to schedule at 815-382-1716 or Sergio at 331-210-2062. Sent email reply thanking him for response and letting him know Planner will reach out to schedule.
05/18/2026 ARoosa - Recvd call from 847-535-7382 Sergio from Lake Forest Biomed, responding to my email stating he thinks they may still have devices and is going to check in surgery. he asked me to send device letter again as well and he will respond to email if he has devices or if they retired devices.

05/11/2026 ARoosa - Sent fu email to Jon at : jonathan.estanislao@gehealthcare.com as well as lakeforestbiomed@gehealthcare.com asking Jon to see if he is POC for this location as well or if he can provide a contact so they can confirm if these devices have been retired.
