We have no record of it being taken out of service, but I have searched each OR and checked our anesthesia machines and cannot locate it. My fear is that someone stuck it in a drawer somewhere.) I sent a reply email stating that we can go ahead and proceed with closing out WO by having him sign paperwork since device can't be located and if he comes accross it in the future to reach out to us and we will get a new WO created and take care of remediation.
