Reached Cardiology. Spoke with a rep and she advised that she was not familiar with the device. She also adv that they moved from 27005 76th Ave New Hyde Park, NY 11040 to 1111 Marcus AveNew Hyde Park, NY 11042. WIll reach out to the DSM for contact info 

06/19/2026 Rbailey - Called 718.470.7000 Advised that I was looking for Adult Cardiology and not Peds Cardiology. Was Xfer'd to a girl in the Cardiology Call Center. She advised she is at 1111 Marcus and she did not know Monique since she was in the Call Center
