06/16/2026 MKlose- rcvd email from Je'Nelle, she adv I have read the letter, and I am not stating that I am unwilling to schedule the PM, I just asked if Bill Burnett can be the one to perform the PM.
If you are not able to provide him, then please send over new dates for Friday availability. Explained I can not schedule a philips FSE. But Philmon could do it Friday August 14th or Friday August 28th,

06/04/2026 mklose rcvd email from POC, she adv this pm was done in March. upon research it was a different serial number. this device was done in September 2025.
06/03/2026 YEspino Called 848-932-9525 to speak to Emily over the pm in question. Was unable to speak to anybody in the Kinesolgy dept so i left a message.
