Notes: FAILING BACKREST GAS SHOCK, LEAKING HYDRAULIC FLUID FROM BOTH LINES, PR LOWER BEARING CARRIER AND BEARINGS DAMAGED; NEEDS (1) 6500-031-077 BACKREST GAS SHOCK, (1) 6500-001-210 HYDRAULIC HOSE, (1) 6500-001-211 HYDRAULIC HOSE, (1) 6500-001-328 LOWER BEARING CARRIER, (2) 6500-101-327 BEARING HALF SHELLS, (1) Mobil 1 HYDRAULIC FLUID, QT Disinfect and clean cot, mattress, restraints, and accessories.
Inspected cot to confirm the product is free from pinch or cut hazards. All welds and assemblies are intact, hardware and wires are properly secured.
Cycle tested cot in all positions, in both power and manual backup mode.
Loaded and unloaded cot with fastener in both power and manual mode.
Cot fully operational, reviewed inspection results with customer, returned cot to service.
Noticeable physical damage on device Follow-up required. Parts Needed- Emsar to follow up with quote for needed replacement part.

12/23 MOwens Dan replied back that the week of 1/5 works better for him. Replied back confirming the schedule for 1/5 with arrival between 9-10am.
12/22 MOwens Sent email to Dan requesting to schedule the visit for the first open date on the schedule for 1/2/26.
12/4/2025 Sramen - pushing out due date since we are still waiting on parts for WO-01315560.
11/26/25 RBerry- Spoke with Dan Haggerty states he would like all WOs to be completed together. Requested that we do not update until tracking is received for WO-01315560 and all are ready to schedule. See logged call on WO-01315552
