6/30/26 RBerry- Placed the work order on hold due to contact and confirmation issues. The ASC has been attempting to schedule the appointment since January but has been unable to obtain confirmation.

4/29/26 RBerry- Sent email to DNakoff asking if there is an update on the clearance for the remaining sites. Per last email from Centurion they were working on setting up a meeting to discuss. See logged email on WO-01316289
4/6/26 RBerry- Tierra Hopps emailed asking for a scheduled date to get entrance approvals. Due to issues with getting clearance and approval sent email to service manager asking for assistance as we want to avoid blocking more time on the calendar until clearance is received. 3/30/26 Rberry- Updated due date. ASC is still waiting on clearance from site. Chattered DNakoff to make him aware.
3/30 MOwens Called to speak to Tierra Hoops. Left a vm for her to call back or email back with update on entry approval for tech.
3/30/26 RBerry- Sent teams message to ASC to follow up with Tierra Hoops today and remaining initial inspections and clearance for these sites. Once this is done due date will be updated.
3/11 MOwens Called and spoke to Tierra Hoops. She is going to check about the issue with approval and send an email with an update.
3/11/26 RBerry- Updated due date. Waiting on background approval.
3/4 MOwens Sent follow up email to Tierra asking for update on approval.
2/20 MOwens Tierra replied back no approval yet. I gave possible date of 3/18 eta between 9-10am.

2/7/26 RBerry- Chattered: @Melissa Owens I see this work order was placed in Reschedule status on 1/27/26, but I’m not seeing any logged notes or communication indicating the reason or confirming the customer was notified. When you have a moment, can you please update the work order with the details?
1/15 MOwens Spoke to Tekeytha and she wants to reschedule the visit for 1/28 with arrival between 2:30-3pm. She sent an email asking for the list of tools and the DL photo be sent to her for the clearance process.
1/12/26 RBerry- Spoke with Tekeytha Fullwood, scheduled for 1/16/26 with arrival time between 9-11AM. Advised that technician has been cleared on background check. Asked if anything else is needed. She requested that tech send a list of tools and cell phone information so that can be cleared ahead of time. Tech will need to bring his photo ID.
1/9 MOwens Sent email to Anitra Hernandez with proposed schedule date of 1/16
12/18/25 RBerry- Received email from Anitra Hernandez with background check form stating: I have attached a clearance form that needs to be filled out for each person that is coming in so they can do a background check. Also, I need a list of the equipment that you are bringing in and how many you will be bringing in. If you need to bring in a phone, I will need some information on that as well, which is the make& model of the phone, Phone #, serial#, IMEI#, the color of the case if you have one on it. Need to know whether or not it's a work phone or personal phone as well.
One more thing, let me know what day you would like to come, & the time.
Forwarded email to Michael Harris and Matthew Bateman asking to complete background check. Also advised MBateman that if he wanted another tech cleared to please let me know. Emailed Anitra back advising this has been sent to the technician to complete and as soon as we receive clearance we will reach out to schedule dates and times. See logged email on WO-01316329
