1/15 MOwens Called and spoke to Tara Kelly. Scheduled the repair visit for first opening for 1/16 with arrival between 8-9am.

1.12 M. Bateman Tech called and said the facility was closing for the day and he would need to return to complete the repair. Placing WO in reschedule.

1/8/26 Per a teams message from M Owens: I just spoke to POC for this WO and they received the additional parts from Midmark. This WO is in awaiting parts. Please put back into RFS for me to schedule for next Monday. 


1/8 MOwens Tara called in and said she received the part. She said she can schedule for Mon, Thurs or Fri. I offered the first available next Monday from 2-4pm. She said that works for her.

12/10 MOwens Called and spoke to Tara Kelly. I offered the first available to schedule on Monday 12/15 with arrival around 1pm. Tara said that date and time works for her for the return visit.

12/9 MOwens Customer called Midmark and this unit is still having a problem with the foot function. Reopening this WO.

12/1 MOwens Called and spoke to Tara Kelly. She requested a Monday, Thurs or Friday morning for the visit. I told her the first available is on Mon. 12/8 around 9am. She said that works for her schedule.

🔧 Work Performed
Check out foot issue. Intermittent issue when raising and lowering feet. Removed seat top. Removed seat top plate. Removed rear control board cover. Inspected wiring. Wiring looks good. Foot will stay stuck in top position and chair needs to be reset. Determined probable limit switch issue. Called Midmark. Spoke with Jason. Tested voltage on board. 2.59vdc. Within spec. Tested limits of foot. Limit switch is not adjusted correctly. Adjusted foot limit switch. Foot section functions as it should. Replaced covers. 12/15/2025. Checked in with customer. Foot section was working intermittently. Started to troubleshoot issue. Removed covers from seat and control board. Foot would not go up when it was in the down position. Called Mdmark. Spoke with Deanne. Tested wiring on J11. Board is good. Tested main harness wiring. Main harness is bad. Part number 015-1937-00. Ordered through Midmark. Foot section would not leave top position. Reversed wires on board to put back to operating position. Replaced covers. 12-29-25. Installed new main wiring harness. Tested chair. Foot function did not operate. Adjusted limit switches. Still no operation. Checked all wiring. Called Midmark. Spoke with Dale. Problem was diagnosed as a bad main PC board. Part number 002-1069-00. Ordered through Midmark. 1.12.26 cag REPLACED MAIN PC BOARD; STARTED TO CALIBRATE AND FAILED WITH CONTINUOUS BEEPS UNTIL UNPLUGGED; STARTED DX: BOTH SWITCHES UP TO CONFIRM HAND PENDANT WORKING, ALL BUTTONS PASS; STOW SWITCH TESTS GOOD; POSITION SENSOR VOLTAGES GOOD AT SENSOR AND BOARD; CHECKED HARNESS WIRING FOR CONTINUITY - NO ISSUE FOUND; POSSIBLE ISSUE AT FOOT ACTUATOR LIMIT SWITCHES -- ALL POSITIONS OF SWITCHES SHOW OPEN AT NUMBERED '4' PIGTAIL; TRIED TO CONTACT MIDMARK BUT WAS CALLER 4 FOR > 30 MINUTES; LOCATION CLOSED AT 1630. SPOKE TO COORD AND SUPRV. // FOOT MOVED UP 45 DEGREES ONE TIME WHILE ON SITE 1.16.26 CONFIRMED BREAK / PEN N WHITE LIMIT SWITCH WIRE FOR FOOT ACTUATOR, CORRECTED, CALIBRATES AND TESTED GOOD
