12/4/25 MFranks If you read the notes on page three it states that unit V2786622 was serviced on a previous work order, that's why there is no service date listed. The only unit serviced on this work order was unit V2786703 and the SR number is correct. I did add the second service date on the signature page. I hope this helps.

12/4/25 NSuarez The SRA MM 9.12.25 for Serial number V2786622 has the Wrong Service date on PG 1 and 3 also the attachment doesn't match the Customer case number please fill out the new SRA that has the correct WO number and Service request #.

08/20/2025 JRolston - Called site to reach site POC, Jacqueline Bouvier @ (866) 234-8534 x2542. Voicemail only. Left detailed message advising her that WO has been received, no parts shipping at this time, and will receive call from coordinators to schedule accordingly.

🔧 Work Performed
11/19/25 AMW - Located autoclave and confirmed SN. POC stated that they had not received any packages for this unit. I called the connectivity team at Midmark. I was put on hold for 30 min before I was able to speak to the connectivity team. She confirmed that the USB module was sent in July. I was on site to do a different SN repair on 9/12 WO-01300427. That package did not have two USB kits but two connectivity modules which were sent back to Midmark per Tyler on the Midmark connectivity team. The packing slip on the original work order is documented and only shows 1 USB kit. Midmark is going to send new USB kit and I will have to return to install. Putting parts placeholder in for parts order. Notified POC of equipment status. 12/2/25 AMW - Received new connectivity module and USB kit. Installed both onto autoclave. Performed function test and EST. Both tests passed. Confirmed autoclave is connected to network. Notified POC of equipment status.
