06/25/2026: ARoosa - Called 602-933-5437 Phoenix children's East Valley Spoke to Pam the operator and I asked her if each campus has their own biomed team or if they only have 1 main biomed and she said they only have biomed at Arrowhead campus and at the Thomas campus. She asked if i wanted to xfer to one of them I told her no thanks that I had already spoken to both of them trying to find this device.
06/25/2026: ARoosa - Called 602-933-5437 Phoenix children's Ambulatory hospital building, xfer to biomed spoke to Alex and he checked for the serial number in active and retired data base and was unable to locate it at his facility.
06/15/2026 ARoosa - Sent fu email to kdy@phoenixchildrens.com asking if he could please let us know if there are any other contacts at the other locations we could reach out to and check for the devices at their other locations as well as asking for reply email stating what he had told me last week about the new facility in the works of being opened and not being able to check if devices are located their until it gets opened and equipment gets logged.
06/08/2026: ARoosa - Called 602-933-1000, xfer to biomed, spoke to Ken he said they have not been able to locate these devices however they are in process of building a new location in Gilbert AZ. and it's possible they may have been shipped to that location but until facility is complete they will not have inventory accounted for. Ken also stated that they have many Phoenix childrens hospitals and it is possible they could be at any one of them. I asked him to send us that info in a reply to prior email sent so we can send of to have it reviewed by Faddy for either ANR or for a delay.

05/28/2026: ARoosa - Called 602-933-1000 xfer to biomed, Spoke to Ken who was super nice he tries to look up one of the SN's and was unable to pull anything up but stated it may be located at a different site and he will physically check for it and he asked me to email him a list of the devices so he can check for the others as well. Sent email to kdy@phoenixchildrens.com
05/14/2026 YEspino Called 602-933-1000 to verify the devices in question. Asked to be transferred over to the biomed dept, but was unable to speak to anybody so i left a message.

04/22/2026 YEspino Called 678-592-8058 to speak to Darlene Mcdowell to verify the devices in question. Was able to speak to Darlene but she advised that she doesnt assist with these devices.
